Application Letter

Congratulations on choosing one of the most progressive intervention services designed to help stop interpersonal aggression and abuse in the nation. The Alternatives to Domestic Aggression Program (ADA) Program has been recognized in the State of Michigan and nation as an innovator and leader in working to end domestic violence since 1986. We look forward to working with you.

Included in this packet are all of the materials you need to apply to the ADA Program of Catholic Social Services of Washtenaw County (CSS). CSS is a non-denominational agency that offers services to people of any, or no, religious affiliation.

To begin the ADA Program you will need to attend an Orientation meeting.

Orientation Dates for 2007

FOR MEN:  We offer Orientation meetings approximately twice per month. The Orientation meeting begins at 8:30 a.m. and ends at 4:30 p.m. It is important that you arrive on time. Please come to the next Orientation meeting that is available. After choosing the Orientation date, write it down at the top of the Application For Service form, so we will know when to expect you.

Program Tuition

Tuition for the ADA Program is based on a sliding scale related to your income. Your tuition amount will be assessed and discussed with you at the Orientation. You are required to bring documentation of your income to the Orientation meeting. You can bring a copy of one of the following: Most recent paycheck stub, W-2, W-4, bank account statement, or other documentation of your income.

Orientation Fee:
The Orientation fee is $50.00. This fee will reserve your place in the meeting. The fee is non-refundable and will be used for the date you have indicated on the Application for Service form. If you need to change the date for your meeting, please let us know at least 24 hours in advance. If you do this, your fee may be transferred. If you do not attend the meeting you were scheduled for and/or do not cancel/reschedule with at least 24 hours notice, your fee will be forfeited.

Application for Service

(Be sure to check the appropriate box at the top of this form indicating the date of the Orientation.

Please fill out the "Application for Service" COMPLETELY (front and back). Send it back to our office within five (5) days, along with your tuition. You may send the application to us at dgarvin@csswashtenaw.org as an attachment to an email. You may also fax it to us at 734.971.2730

This is a listing of the dates of the upcoming Orientations. Please select the next date of the appropriate meeting and write that date at the top of your Application for Service.

This map shows how to get to our office. It is wise to write the date of your Orientation on this map as a personal reminder.

ADA Accountability Workbook

You are required to purchase the "ADA Accountability Workbook". The cost of this workbook is $25.00.

You may want to call to confirm that we have received your application before you attend the Orientation Meeting. Problems with the mail may delay our receipt of your Application.
